All Fees are quoted in Australian dollars (AUD$) and are inclusive of 10% GST (Goods and Services Tax). Registration after 5 May 2011 must be made at the conference registration desk.

*Alzheimer’s Australia is seeking sponsorship to assist people with dementia and their accompanying family carers to attend the conference at a subsidised rate. Further details will be provided when available. Please contact the conference secretariat for more information.

** Students must provide a copy of their valid student ID card as proof of their status to the conference secretariat

*** Speakers attending to present their sessions only will be complimentary. Speakers wishing to participate in the conference outside their speaking segment are requested to register in accordance with the category rates above.

• Congress welcome function (Tuesday) The conference welcome function is a ticketed event; you must indicate attendance via the registration form.
• Congress dinner (Thursday) The conference dinner is a ticketed event, you must indicate attendance via the registration form.

Day registration:

• Attendance at all sessions and exhibition
• Satchel including program
• Morning tea for the day registered
• Lunch for the day registered
• Afternoon tea for the day registered

You are welcome to purchase social function tickets at an additional cost.

Method of payment

Payment of fees must accompany all complete registrations. Registration will not be confirmed until full payment is received. Payment can be made by the following methods:

• Credit cards (Visa, MasterCard, American Express and Diners)
• Cheque in Australian dollars payable at an Australian bank and free of any bank charges*

*Cheques should be made payable to Alzheimer's Australia 14^th national conference 2011

Please note:

• Debits to your credit card, excluding accommodation, will appear as ICMS Australasia on your credit card statement.
• If you are making a credit card payment from a country outside Australia, for security reasons you must first advise your banking institution of the impending charge and authorise them to allow the charge from Australia to go through for up to 48 hours (this is due to the time difference between our countries).

Registration cancellation policy

Cancellations must be notified in writing to the conference secretariat. Cancellations made before 1 April 2011 will be refunded less $150.00 to cover administration costs. Refunds for cancellations received on or after 1 April 2011 will only be made in exceptional circumstances. As an alternative to cancellation, your registration may be transferred to another person on or before 1 April 2011 without incurring any cost penalty. The conference secretariat must be advised in writing of any alterations or transfers.

The organising committee and the conference organisers are not liable for any loss or inconvenience caused as a result of such alteration. In the event of unforeseen circumstances, the Alzheimer's Australia 14th national conference, organising committee and the conference organisers do not accept responsibility for loss of monies caused by delays.

Participants are advised to take out personal travel insurance and to extend their policy to cover personal possessions. The meeting does not cover individuals against cancellations of bookings or theft or damage to belongings.
